We all go through transitions from time to time. As for children with disabilities, transition is part of the individualized educational program, which determines the transition from school to adult life. An Individualized Educational Program (IEP) Transition Plan is required by law  <\/span><span style=\"font-weight: 400;\">(IDEA),<\/span> <span style=\"font-weight: 400;\">for students with disabilities.<\/span><\/p>\n<p><b>What is the transition plan?  <\/b><\/p>\n<p><span style=\"font-weight: 400;\">The law  <\/span><span style=\"font-weight: 400;\">Governing the Individual Transition Plan is the Individuals with Disabilities Education Act (IDEA), Therefore, transitional services should be available as part of the Individualized Educational Program (IEP). This includes the team assessing the child and setting goals in the following areas:<\/span><\/p>\n<ul>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Training<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">education  <\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">recruitment  <\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Live independently<\/span><\/li>\n<\/ul>\n<p><span style=\"font-weight: 400;\">Schools must provide transitional services that in turn lead to achieving these goals, These goals should be set by the time the child reaches the age of 16 years. This means that planning for the transition plan actually begins before the child turns 16. In some states, The IEP transition plan may be made at an early age. Example, The state of Texas requires that a transition plan be planned by age 14.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">The student&#8217;s needs, related goals, and action plan to achieve those goals should be documented and kept on file at the school. Parents should also be informed of the progress the student is making toward meeting those goals in the transition plan.<\/span><\/p>\n<p><b>Why is a transition plan important in the field of special education?<\/b><\/p>\n<p><span style=\"font-weight: 400;\">All students need guidance in order to transition from high school to post-secondary school. However, Students with disabilities may need more assistance in post-secondary education. Therefore, an IEP transition plan may serve to ensure not only that these children will be able to function as adults in the future but also increase the likelihood of pursuing post-secondary education.
